This site does not have a third-party analytics vendor, advertising pixel, or newsletter form. The hosting environment may process ordinary request information needed to serve the site.

Requesting contact from Nexopic

When you submit the optional contact request, we collect your name, email, phone, preferred contact method, state, ZIP code, household type, adult age range, budget range, timing, and the two consent choices shown on the form. We also record the submission time, consent wording/version, request reference, landing-page path, and recognized campaign labels. Please do not send medical history, diagnoses, or payment information.

Nexopic receives this information to respond to your healthshare research request. Cloudflare hosts the site and temporarily stores submissions for reliable delivery. Our advisor team at Nexopic uses your inquiry to discuss your options and may manage it in GoHighLevel. Program profiles are not a list of commercial recipients.

Retention and choices

The delivery system schedules removal of contact details and questionnaire payloads after 30 days. A submission hash, reference, delivery state, and CRM contact identifier remain for up to 90 days to help reconcile retries. Cloudflare backup retention may extend recovery copies beyond active-record deletion. Nexopic manages the CRM record after delivery; contact Nexopic for its retention policy or a request to access, correct, delete, or stop using your information.

A signed receipt cookie expires after 10 minutes. Rate-limiting uses a keyed hash of the request IP address, with temporary counters removed by the delivery cleanup job; the integration does not store raw IP addresses in its lead database. Contact details and answers are not saved in browser storage or analytics.

You can ask your advisor to update or withdraw your request when they contact you. Keep your request reference for future correspondence.

Using the tools

Search and comparison controls operate in your browser. A shared comparison URL contains program identifiers. The educational next-step tool keeps answers in page memory and does not save them in browser storage. Refreshing the page clears them. A separate session-storage record retains only an allowlisted landing page and recognized campaign labels for attribution; it contains no contact details or questionnaire answers.

Local demonstration

A separately enabled local demonstration uses fictional, fixed contact details. It sends those test values to the local server, sets a short-lived demonstration receipt cookie, and sends nothing to a partner. Do not enter real contact or medical information into that demonstration.
